The deadline date for the receipt of the School of Public Health application is June 15 of every year; if that deadline has passed, contact Carole Bland, PhD, fellowship director, and she will handle your request.
You will be applying to two places:
1. University of Minnesota School of Public Health
Applicants must submit the following:
- Completed School of Public Health Application for Admission and application fee
- Letter of intent describing professional experience, career goals, and type of setting in which you plan to work
- Resumé or CV
- Copy of transcripts from each college/university attended
- Three completed SPH recommendation forms with attached letters of reference from persons qualified to assess your academic work; clinical, public health or professional experiences; or leadership potential in public health (one of the three required recommendations should be from the clinical director of training supporting your potential as a clinical researcher)
- TOEFL test scores (international applicants)

2. Department of Family Medicine and Community Health
Applicants must submit the following:
- Copy of the School of Public Health application
- Copy of letter of intent
- Resumé or CV

- Photocopy of all license certificates or wallet cards
- Application for Family Medicine Fellowship in Clinical Research (MS Word). Print and fill out.
